Making a Closed Bed

Preparation

WASH HANDS AND WEAR GLOVES

Collect Clean Linen: (Soaker Pad, Fitted Sheet, Drawsheet, Top Sheet, Blanket, Bedspread, Two Pillow Cases) Gloves, Laundry Basket

Place Linen on Clean Surface

Laundry Basket near bed

RAISE BED TO COMFORTABLE WORKING HEIGHT.

Procedure

 

Use Gloves with soiled linens (blood and body fluids)

Remove Dirty Linen. ROLL each piece away from you (do _not_ shake linens) and place it in the laundry bag.

Discard soiled gloves. WASH HANDS.

Unfold Fitted Sheet Lengthwise

Cotton Drawsheet in middle of bed

Soaker Pad Under Clients bottom

Top Sheet Face Hem-Stitching Downward away from Client (Open sheet by unfolding lengthwise with centre crease in the middle of the bed) FANFOLD it toward the other side of the bed. Do _not_ shake linens. Tuck the Top Sheet under the mattress (according to CARE PLAN _and_ employer policy). MAKE SURE THE SHEET IS TIGHT AND SMOOTH. Hospital corners on the tucked sheets.

Ensure sheets are smooth and wrinkle free.

Place Blanket on the bed unfolded so that the centre crease is in the middle of the bed. Fanfold blanket to opposite side. Hem stitching should be facing down and away from the client.

Bedspread on bed unfolded so centre crease is in the middle. Fanfold to opposite side.

Tuck in all sheet under mattress (according to CARE PLAN and employer policy).

Ensure all linens are SMOOTH AND WRINKLE FREE.

Place Pillow at top of bed

Open the pillowcase so that it is flat on the bed

Fold pillow in half lengthwise while inserting in to pillowcase (the open end should be away from the door)

Post-Procedure

 

***ENSURE THE CALL BELL IS WITHIN REACH.

***LOWER THE BED TO ITS LOWEST POSITION ENSURING THE WHEELS ARE LOCKED.

REMOVE SOILED LAUNDRY BAG ACCORDING TO EMPLOYER POLICY.

WASH HANDS.

 


 

 

Making an Occupied Bed

Preparation

#1 Identify the client, according to the employer policy.

#2 Explain the procedure to the client.

#3 Wash Hands and Wear Gloves when necessary.

#4 Collect Clean Linen: (Soaker Pad, Fitted Sheet, Drawsheet, Top Sheet, Blanket, Bedspread, Two Pillow Cases) ** ADD BATH BLANKET

#5 Place linen on clean service.

#6 Provide for Privacy.

#7 Place laundry bag near the bed.

#8 Raise the bed to a comfortable working height. FOLLOW THE CARE PLAN FOR BED RAIL USE.**

#9 Lower the head of the bed to be as flat as possible. Lower bed rail near you if it is up.

Procedure

#10 Put on gloves when linens are soiled with blood and body fluids.

#11 Loosen top linens at the foot of the bed.

#12 Remove the bedspread and blanket separately. (Fold linen lengthwise down and neately when being reused.)

#13 Cover the client with a bath blanket to provide warmth and privacy.

  • Unfold bath blanket over the top sheet.
  • ASK THE CLIENT to hold on to the bath blanket over the top sheet. (**If the client is not able to do this, tuck the top part under the client's shoulders.)
  • Grasp the top sheet under the bath blanket at the shoulders. Bring the sheet down to the foot of the bed. Remove the sheet from under the blanket. **Can be accomplished by pulling the top sheet from standing at the foot of the bed.
  • **If a bath blanket is not available, the existing top sheet can be used if it is not soiled or wet.

 

#14 Move the mattress to the head of the bed.

#15 Position the client on the side of the bed opposite to where you will begin changing the bed. Adjust the pillow for comfort.

#16 Loosen bottom linens from the head of the bed to the foot of the bed (TOP TO BOTTOM, CLEANEST TO DIRTIEST).

#17 FANFOLD bottom linens one at a time toward the client. Start with the COTTON DRAWSHEET. (When reusing the fitted mattress sheet, do not fanfold it.) CHOOSE NOT TO SHAKE LINENS WHEN CHANGING A BED TO ENSURE CLEAN STANDARD PRACTICE.

#18 Place a clean mattress pad on the bed. Unfold it lengthwise, with the centre crease in the middle. FANFOLD the top part toward the client. If you will be reusing the mattress SOAKER pad, straighten and smooth any wrinkles.

#19 Place the bottom sheet on the mattress pad, with the hem stitching away from the client. Unfold the sheet so that the crease is in the middle and the small hem even with the bottom of the mattress. FANFOLD the top part toward the client.

#20 Tuck the sheet under the mattress from the head to the foot.

#21 Raise the bed rail if it was used. Go to the other side of the bed and lower the bed rail.

#22 Explain to the client that he or she will roll over a bump. Assure the client that he or she will not fall.

#23 Help the client turn to the other side (using proper body mechanics: ASK CLIENT TO bend closest knee, then ask them to pull themselves toward the bed rail in the direction they are turning). Adjust the pillow for the clients comfort.

#24 Loosen the bottom linens. Remove one piece at a time, and place each piece in the laundry bag.

#25 Remove and discard the gloves and WASH HANDS.

#26 Straighten and Smooth the Mattress Pad.

#27 Pull the clean bottom sheet toward you. Tuck the sheet under the mattress from the head to the foot of the bed.

#28 Pull the drawsheets tightly toward you. Tuck both under together.

#29 Position the client supine in the centre of the bed. Adjust the pillow for comfort.

#30 Put the top sheet on the bed. Unfold it lengthwise. The crease is in the middle, and the large hem is even with the top of the mattress. Hem stitching is on the outside.

#31 ASK client to hold on to the sheet so that you can remove the bath blanket. OR tuck the top sheet under the client's shoulders.

#32 Place the bath blanket on the bed so the upper hem is 15cm from the top of the mattress.

#33 Place the bedspread on the bed. Unfold it so the centre crease is in the middle and it covers the client. The top hem is even with the mattress top.

#34 Go to the foot of the bed.

#35 Make a toe pleat one hand length from the foot of the bed. This is done to ensure that circulation is not cut off on the client's toes while they are in bed. Less pressure on the feet helps to prevent skin breakdown complications.

#36 Lift the mattress corner with one arm. Tuck all linens under the mattress together.

#37 Raise the bed rail and go to the other side to lower the bed rail.

#38 Straighten and smooth top linens.

#39 Tuck the top linens under the mattress.

#40 Change pillowcases and raise head of the bed to level appropriate for the client (and use pillows to position the client).

Post-Procedure

 

#41 Provide for safety and comfort.

#42 Place the call bell within reach.

#43 Lower the bed to its lowest position. Follw the care plan for bed rail use.

#44 Remove soiled linen bag from the room and follow the employer policy for dirty linen.

#45 Wash hands well.
