Polyenes

 

Amphotericin B

creates pores in the fungal cell wall

Azoles

Also act on cell wall.

Interact with cyt P450.

Fluconazole

  • good against C. albicans and the majority of yeasts
  • C. krusei, C. glabrata show resistance
  • resistance can develop
  • IV and oral
  • well tolerated

 

Itraconazole

  • active against Aspergillus

Voriconazole, ravuconazole, posaconazole

  • wider spectrum
  • Aspergilus,
  • can cause visual disturbances in 30%
  • QT prolongation
  • IV formulation in

Echinocandins

Caspofungin

Inhibit glucan synthesis

use if renal impairment
